If you still have the app, you can delete your account there yourself: Profile → Delete account. That takes effect immediately. This form is for anyone who can no longer get into the app.
On deletion, your profile, your pictures and your messages are removed. If a ban has been imposed on you, only the telephone number remains stored for as long as the ban applies – solely so that it cannot be circumvented by registering again.
